Vein Care of New York works in conjunction with many health insurance plans for vein treatments which are deemed medically necessary. Please note that each individual plan may vary with respect to coverage and reimbursement guidelines. We encourage you to complete the adjacent form or call us at 1.800.691.VEIN (8346) so we may verify your coverage benefits prior to your appointment.

Cancellation Policy:

At Vein Care of New York, we recognize that your time is important and schedule your appointments according to your convenience. We reserve adequate time for you to be seen and meet with the doctor so that all of your questions or concerns are addressed.  We kindly request that you consider our commitment to the schedule as well by providing us with at least 48 hours notification should you need to reschedule an appointment.
